  • Trailing lead across the walkway. A trip hazard on the main route. Route cables safely or cover them — don’t leave them across a path.
  • Missing guard rail. An open edge on the scaffold is a fall risk. That section should be stopped and fixed before anyone uses it.
  • Visitor with no PPE or sign-in. Everyone on site must be signed in, inducted and wearing the right PPE — no exceptions, including visitors.
  • Fire exit blocked. Rubbish in front of a fire exit is dangerous and illegal. Escape routes must be kept clear at all times.
  • Keys left in unattended plant. Machinery left with the keys in can be started by anyone. Isolate and secure plant when it’s not in use.
  • No method statement for live work. Work happening without its method statement (RAMS) on site shouldn’t be going ahead — stop and sort it.
