Are all your operatives trained for the tasks they're doing?
Including mandatory tickets — CSCS, CPCS, asbestos awareness, working-at-height etc.
How often do you run toolbox talks or briefings?

Why these five areas?
Most construction-site H&S problems boil down to one of five things: the people, the equipment, the planning, the paper trail, or the place itself. Strong sites are strong across all five — weak sites usually have one obvious gap that lets the others down. Scoring by area shows you exactly where to focus. There’s no point investing in fancy permit-to-work software if your operatives aren’t trained to follow it; equally, the best-trained team in the world can’t work safely if the welfare facilities are non-existent.
The scoring is deliberately strict — full marks means “exemplary, would-show-an-HSE-inspector”, not “good enough”. That’s why a 70% score is genuinely good, and 80%+ is exceptional. If you score lower than you expected, that’s the tool doing its job: surfacing the gaps you can fix before they cost you.
